Pretty easy, Here is what you need.
1. Macro generator, go to KMAConnect, login. do a search for this, download it and install it on your PC. In the setup, run it twice for B/W and Color. (This creates a print driver for creating macros)

2. Download IC-Link from KMAconnect and install. (this will create the CF card with the form on it.)

3. Create a form using any program. (Word, Publisher, Adobe)

4. When you have it ready, print it using Macro generator, color or BW. Save the macro in a place where you can find it back.

5. Place a CF card in the printer that is going to use it and format the Memory Card. You will also need to setup the KX print driver for that printer on your PC.

6. Open IC Link, hit the icon for "Add Resources" and grab the macro (not macro01 or macro2, why the generator gives you 3 is beyond me) Then go to tools, Multi-Form Generator, select the drawer that want, select Data and overlay and send it to the printer. IC Link will want you to save 2 docs. Save them because you will need them in the next step. (ccpyon.prn and ccpyoff.prn)

Go to your print driver for the printer that is going to have the Form on. Right Click on driver, go to Properties, device settings and hit the PDL button, select PCL 5. Then go to the Advanced Tab, Printer Defaults, last tab will have epolog/prolog button, select it, hit add, Browse Box will open, select the dropdown from .txt to All Files and find the ccpyon.prn and hit ok. With the file path in the epolog box highlighted, click on the "Beginning of the document" radio button. Do this again for the ccpyoff.prn and click on the "End of the Document" and hit ok. Apply this to the driver by hitting Apply and Ok.

7. Try printing a test page on the driver and you should see the form with the Test page info on the same page.

This should be ready to go for printing forms.
